Top Questions to Ask any Stem Cell Clinic

There’ are a lot of “Questions to Ask Stem Cell Clinic” Guides out there.

One thing to remember is Clinics have heard these questions before and have the answers in their marketing materials ready. And it’s really difficult to prove that what they’re saying is legitimate, especially in more developing regions where there are no regulations.

Use these questions to educate yourself, spot red flags and understand what good looks like. But the strongest signal you can get,  beyond anything a clinic tells you, is someone outside of their own network who has been there, had treatment and can vouch for them.

1. What Types of Stem Cells Do You Use?

There are two things you need to understand here. The source of the cells and the type of treatment. These are separate questions and both matter. Cells come from one of three places:

Bone Marrow Taken from your own hip bone
Fat Tissue (Adipose) Taken from your own body via a minor liposuction procedure
Umbilical Cord (Wharton’s Jelly) Donated after birth from a third party donor
Expanded (Cultured) Cells are grown in a lab to produce a high, controlled dose of MSCs
Minimally Manipulated Cells go from source to treatment with very little processing

These are fundamentally different treatments with different risks, different limitations and different expected outcomes. 2. If You’re Using Umbilical Cord Cells, How Are They Sourced?

This question is only relevant if your treatment involves umbilical cord derived cells. Umbilical cord cells come from a donor, which means you have no control over where they come from or how they were handled before reaching you. These are the things you need to ask:

Ask the clinic
Are the cords donated with informed consent from the mother?
What infectious disease screening has been done on the donor tissue? This should include HIV, hepatitis, syphilis and CMV at minimum.
Are the cells fresh or frozen? If frozen, how does the clinic test that the cells are still viable after thawing?
Can they provide a certificate of analysis for the donor tissue?

Remember the opening point here. A clinic can answer all of these questions confidently. The reality is that by the time you are reviewing any documentation, you have likely already travelled, paid a deposit and committed to treatment. Verifying whether a certificate from a clinic in another country is genuinely legitimate is extremely difficult. This is why doing your research before you travel, not after you arrive, is so important.

3. How Are You Testing Cell Viability Before Treatment?

Viability refers to the percentage of cells that are actually alive at the point of treatment.

Stem cells work because they are alive and active in your body. If a significant percentage of the cells are dead or damaged before they even reach you, the treatment cannot do what it is supposed to do. It’s important for you to understand the importance of getting cells that are alive and working.

4. Can You Confirm the Cells Are Genuine MSCs Using CD Markers?

Most people have never heard of CD markers, but they are how scientists actually verify what type of cell they are working with. Think of them as ID tags on the surface of the cell.

For mesenchymal stem cells, the cells should be:
Positive for CD73, CD90 and CD105
Negative for CD34 and CD45

Without this testing, a clinic cannot reliably prove that what they are giving you is actually MSCs and not a mix of other cell types. The reason this question matters is not really about catching out a bad clinic. It is about you making sure you’re getting actual Mesenchymal Stem Cells and what clinics are talking about when they mention “Checking markers”.

5.  What Is the Full Price of Everything, Including Consultation, Scans, Procedure, and Post-Care?

Detailed Breakdown Ask for a comprehensive list of all associated costs, including the initial consultation, diagnostic scans, the procedure itself, and any post-care or follow-up visits.
Hidden Fees Inquire about any additional fees, such as laboratory charges, medications, or equipment costs.
Accommodation and Extras Does the price include things like accommodation, translators or is that extra?
Additional Tax Payment In some countries like Japan, there will be an extra 10% added on for tax consumption. Make sure the price you have includes this. Always ask!

Make sure you get this all in writing before booking any treatment. There have been cases in the US where clinics have been accused of high pressure sales tactics and offering 0% interest payment plans that turned out to be loans carrying annual percentage rates of 9.99% or higher.

6. Do You Guarantee Results?

The answer should always be no. Stem cell therapy outcomes depend on the condition being treated, its severity, your age, your overall health and how your body individually responds. No clinic and no doctor can predict with certainty how you will respond. Anyone telling you otherwise is selling, not informing.

Clear explanation of how success is measured and over what timeframe

Stem cell therapy can deliver meaningful results for many people, but the science is not at a point where outcomes can be guaranteed for any patient. 7. What Qualifications and Experience Does the Medical Staff Have?

Things to look at here:

Are they actually a registered doctor? If you are travelling abroad for treatment, find out how to verify the doctor is genuinely registered to practice medicine in that country. Most countries have a public medical register you can check online. If you cannot find a way to verify them, that is a problem.
Are they the right kind of specialist? If you are being treated for an orthopedic condition, you should be seeing an orthopedic specialist. Not a cosmetic specialist who has added stem cell therapy to their service list. Specialty matters because the doctor needs to understand your condition, not just the cells.
Do they have experience with the specific procedure you are receiving? Stem cell therapy is not one procedure. Delivering cells into a facet joint in the spine, or via an intrathecal injection vs a Stem Cell Facial are completely different skills. A doctor inexperienced in the specific delivery method can cause serious complications, and this has nothing to do with the quality of the cells themselves. This is where major side effects can happen.

8. What are success rates for my specific condition?

This is where it gets really difficult, because most clinics will give generic ranges. So honestly it’s hard to know if you’re hearing the truth here. And you can’t always trust celebrity endorsements here!

For us, we’ve worked with clinics who have told us someone actually will not be a fit, or if the data suggests it’s a long shot. For example, when someone has had severe meniscus damage across multiple compartments, doctors we work with have told us stem cells are unlikely to deliver meaningful results and it would be a long shot.

9. What Is the Clinic’s Accreditation and Licensing Status?

Regulations vary massively depending on where you are being treated. You need to know what the rules actually are in that country and whether the clinic is operating within them. For example:

Japan and South Korea Clinics have to be formally approved to offer expanded stem cell treatments. There is a clear regulatory pathway and you can verify it. In Japan, they will have certificates for each treatment they can offer.
United States Regulations are state-specific. In Florida for example, clinics offering Wharton’s Jelly products have to source from labs that are registered with the FDA and the treatment must be minimally manipulated.
Mexico, Panama, Colombia, Thailand and similar There is little to no enforced regulation around stem cell therapy. The bigger issue here is honesty. For example, Stem cell therapy in Mexico is not legally approved, but clinics will throw around terms like COFEPRIS approval or government endorsement in a way that misleads patients into thinking the treatment is officially sanctioned. Most patients researching stem cell therapy already understand it is experimental. The problem is when clinics try to dress up their legal status rather than being upfront.

A clinic being honest about operating in a regulatory grey area is far more trustworthy than one using official sounding language to make their setup seem more legitimate than it is.

10. GMP Compliance & ISO:9001

There is a meaningful difference between the two. GMP compliant is essentially a self-declaration. The clinic or lab is saying they follow GMP standards internally, but no external body has verified it. GMP certified or accredited means a third party has audited the facility and formally verified that the standards are being met.

When a clinic mentions GMP, ask which one they mean and who certified them. This point is just so you’re aware of the differences.

ISO 9001

Many clinics use ISO 9001 as a quality marker in their materials. It is worth knowing what it actually is. ISO 9001 is a generic quality management system standard that applies to any business in any industry. It is about documented processes, consistency and continuous improvement. It says nothing about cell handling, sterility, contamination control or anything specific to biological treatments.

A clinic using ISO 9001 is not proof of cell quality.

In-house vs Outsourced Lab
Ask whether the clinic operates their own lab or outsources cell collection and processing. Both can be done well, but if the lab is outsourced, you need to know who the third party is and what their standards are. The clinic is ultimately responsible for what they inject into you, regardless of who processed it. Poor lab quality and poor handling of cells is what can lead to severe side effects like infections and viruses. This is where a lot of complications can occur.

11. What Is the Timeline for Seeing Results?

Set realistic expectations for recovery:

Expected Timeline Ask how long it typically takes to see results from the therapy. Across most treatments we have seen, patients tend to notice the first signs of improvement between one and three months after treatment.
Follow-Up Care Inquire about any follow-up care or assessments to monitor your progress.

Why is this important? Knowing the timeline helps manage expectations and prepare for the commitment required. Of course, this will vary from person to person but it’s good to know how other people have reacted to treatment. 12. What Does Aftercare Look Like Once You Are Home?

Most patients fly home within days of treatment. Ask the clinic exactly what their aftercare process looks like once you are back.

Some clinics offer follow up calls or video consultations. Others do not. Either way, you need to understand the reality. If something goes wrong, the clinic has very little practical ability to help you. You will end up at a local doctor who likely has no specialist knowledge of stem cell therapy. And legally, taking action against a clinic in Mexico, Panama or Thailand from the US, UK or Australia is extremely difficult and rarely worth pursuing.

This is one of the genuine trade-offs of travelling abroad for treatment. Go with your eyes open and choose the clinic carefully before you book.


13. Are you dealing with an agency?

This is far more common than most you realise. Many of the websites are run by a third party agency posing as the clinic, branded to look like the clinic, using clinic imagery and answering questions as if they are the clinic when they are not. Often, there can be multiple “brands” using the same clinic.

It is not always a bad thing. Some agencies are there to help bridge the skills or language gap for clinics that don’t have the internal teams or skill sets.

The issue is the misleading presentation.
