How Japan Regulates Stem Cell Therapy

Japan has a three-tiered regulatory system under the Act on the Safety of Regenerative Medicine (ASRM) and Pharmaceutical and Medical Device Act (PMDA), categorizing treatments based on their risk level and level of manipulation. For simplicity, we’ll cover the first 2:

  • Category 1 (Highest Risk):
    • Banned for Commercial Use: Embryonic stem cells (ESCs) and induced pluripotent stem cells (iPSCs) are strictly banned for clinical treatments at private clinics. (page 3 of the MLHW Guidelines).
  • Category 2 (Moderate Risk):
    • Allowed for Treatment: Expanded/cultured autologous stem cells (stem cells from you that have been multiplied in a lab) are allowed for treatment at private clinics with government approval
    • Not Offered by Clinics: Umbilical cord-derived mesenchymal stem cells (UC-MSCs) are not available for patient treatments at private clinics. So, there are clinics in Japan operating without approval. If you’re speaking to any, make sure you ask them if they have the relevant treatment approval. For example, if you’re getting Parkinson’s treatment, they’ll have a specific neurological certificate for this. Is Parkinson’s Stem Cell Treatment approved in Japan in 2025?

No, Stem Cell treatment for Parkinson’s isn’t officially approved in Japan as of August 2025. But some clinics can still offer it as a regulated experimental treatment if their treatment plans have been accepted by the Ministry of Health, Labour & Welfare.

Japan’s national drug regulator, the Pharmaceuticals and Medical Devices Agency (PMDA) has not granted marketing approval for any stem cell product to treat Parkinson’s disease. All officially approved stem cell treatments are on the PDMA’s website.

A PMDA approval means a product has been proven safe and effective for a specific condition and can be marketed nationwide. 

ASRM permission, on the other hand, only applies to the clinic that filed the plan and is more like a hospital being allowed to run a closely monitored procedure. These ASRM treatments are still considered experimental, even though they’re regulated and legal in Japan.
