Does Medicare in the US cover Stem Cell treatments?
Medicare covers your stem cell treatment only if it’s an FDA approved hematopoietic transplant for a qualifying blood or immune disorder. But not for commercial or experimental treatments.
You can read more on the official website of the Centers for Medicare & Medicaid Services
Unfortunately, other types of stem cell procedures are not covered. You have to pay all costs yourself!
Is Stem Cell Therapy FDA Approved?
No, Stem Cell Treatments using Mesenchymal Stem Cells have not been approved by the FDA.
However, there are Hematopoietic stem cell transplants that have been approved by the FDA for blood cancers & blood/immune disorders.
These treatments have gone through clinical trials and received a Biologics License Application (BLA) from the FDA, making them legal and available in clinical practice.
This PDF goes into the FDA’a official stance on stem cell treatments. We know there’s a lot going around now in terms of what’s legal and what’s not, we’re breaking it down for you below.
That doesn’t mean all treatments are necessarily illegal.
What Stem Cell Treatments are Illegal in the USA
Stem cell treatments, including those that isolate, culture, or expand stem cells, are regulated as drugs or biological products. And treatments that use umbilical cord derived products are also illegal (aside from Florida & Utah).
Basically treatments that separate stem cells and grow them are not allowed.
Offering these treatments without FDA review is illegal. The FDA has enforced this position with warning letters, injunctions, and court rulings. But there are exceptions for certain states, as you’ll find out below.
Can Clinics in the US offer Stem Cell Treatments?
Yes, if doctors take your own stem cells out and inject them back in during the same surgery, without growing or changing them, it’s fine. But in Florida & Utah, clinics are allowed to offer umbilical cord derived treatments for certain conditions. BUT they must still be “minimally manipulated”.
The specific FDA rules & legal exceptions (click to read more)
But if the cells are changed, stored, or used for something different than their original purpose, the treatment needs full FDA approval. Below is the more formal wording:
According to 21 CFR 1271.15(b), stem cell treatments do not require FDA review if:
- The patient’s own tissue (or cells) is removed and re-implanted in the same patient,
- The procedure occurs within the same surgical session,
- The tissue is only minimally manipulated (no isolation or expansion), and
- The use is homologous, meaning the tissue’s intended role is the same in the recipient.
The FDA has stated that this exception applies only when tissue is used “in its original form, with minimal processing”. So if your treatment includes taking the stem cells from your own body & re-injecting them straight away, it’s OK.
In Summary
Legal: FDA‑approved hematopoietic stem cell treatments for certain cancers and immune conditions.
Potentially Legal: Autologous, minimally manipulated, homologous tissue administered in the same surgical procedure (21 CFR 1271.15(b)).
Illegal Without IND/BLA: All other stem cell treatments that do not qualify for the “same surgical procedure” exception.

What Do US Clinics Mean When They Say “Off‑Label Use is Legal”?
When US clinics say “off-label use is legal,” they’re often misusing the term. Off-label only applies to FDA-approved drugs used for new purposes, not to unapproved stem cell products, which still need full FDA approval before use.
Deep Dive: Is this considered “Off-Label”? FDA definitions (click to read more)
We break it down here.
“Off‑label” applies only when a doctor prescribes an FDA‑approved therapy for an indication that is not in its labeling. According to the FDA:
(FDA.gov: Understanding Unapproved Use of Approved Products)
However, when it comes to stem cell treatments, the FDA clearly states:
(FDA.gov:Important Patient and Consumer Information About Regenerative Medicine Therapies )
In simple terms, Off‑label means a doctor can use a therapy that the FDA has already approved for one condition to treat a different condition. This is common in medicine. For example, using an approved drug for another illness.
But with stem cell treatments, most aren’t FDA‑approved at all. So when a clinic talks about “off‑label” use for umbilical cord derived stem cells, that doesn’t apply.
These treatments aren’t “off‑label”, they’re unapproved and must go through the FDA’s IND or BLA process.
What other countries offer Stem Cell Treatments outside of the US?
Other popular countries include Mexico, Panama, Colombia, Thailand Japan & South Korea.
Mexico is definitely the cheapest, but may not have the best reputation.
Japan & South Korea are quite a bit further away but Stem Cell treatments in these countries are much more regulated & considered safer. They’re also not that much more expensive either.
South East Asia is also leading the way with research in this area, by a long shot. We’ve put together articles covering Colombia, Japan & South Korea. We’ve also broken down key differences in this table
| Country | Cultured Cells | Regulations | Treatments Available |
| USA | No (Utah is the exception) | Strict | Orthopedic |
| Mexico | Yes | Lax | Wide Variety |
| Colombia | Yes | Lax | Wide Variety |
| South Korea | Yes | Strict | Wide Variety |

For Orthopedic conditions, yes America is an option, especially if you’re living here. If you want umbilical cord derived treatments, you can look at Florida & Utah with costs for orthopedic conditions ranging between $4,000 – $10,000. But remember, these aren’t the same expanded treatments you’re getting in other countries.
To read about how to spot red flags when comparing clinics in the US, check out our Guide on Spotting Red Flags in Clinics in the US .
- Check the medical team. Is an actual doctor with a background in orthopedics treating you? Have any of the medical team been involved in fraud or had charges against them? These are just a couple of things to take note of.
- Sourcing Transparency. If a clinic in Florida or Utah is using products derived from umbilical cord tissue, are they sourcing their cells from an FDA-accredited lab?
- Cell Count Reality. With minimally manipulated cells, it’s difficult to know how many viable MSCs you’re getting per dose. This isn’t necessarily “bad,” but it is a variable you must be aware of.
- Marketing Red Flags. Be wary of clinics that claim miracle cures for wide-ranging conditions.

“Minimally manipulated” means the tissue has only been handled in basic ways and hasn’t been altered or grown in a lab. In simple terms, it’s used pretty much as it came from the body.
For Stem Cell Treatments, it means the cells have not been grown or expanded in a lab.
Looking at actual patient data, Stem Cell treatments for Orthopaedic injuries in Mexico cost around $5,000 per joint.
Regarding all treatments, the rough range is $2,500 to $20,000 depending on the exact treatment you’re getting.
People go to Mexico for stem cell therapy because it’s often cheaper, regulations are looser, and they can access a wider range of treatments that aren’t available or approved in the US.
